Private-Pay Only, Out of Network Provider. We do not accept Medicare/Medicaid.

• $350 Podiatric Medicine House Call, Patient Evaluation, Exam and Consultation.
• $300 Podiatric Medicine In-Office Visit, Patient Evaluation, Exam and Consultation. Saturdays Only at Del Ray office location.
• $700 Emergency House Call:* After Hours, *Sundays and *Holidays (*if doctor is available).

All Foot Care Treatment including Routine Foot Care/Medical pedicures, Injections and other Procedure FEES ARE IN ADDITION to the house call or office visit fee. Please call for rates.

Mileage Fee applicable to long distance house calls.

**$50 Service fee for non cancellation of house call if not notified in 24 hours of scheduled appointment time. ***$150 service fee assessed if the doctor makes the house call and patient wants to cancel without 24 hour notice.

As a private-pay, out-of-network (OON) provider, payment in full is due at time of service. If requested, I will provide the patient with a receipt or an invoice with corresponding medical codes and charges paid via email. It is the patient's responsibility for contacting and submitting paperwork to their insurance company for possible reimbursement. Please note, not all treatments are covered by insurance.

*Medicare Patients: I have opted out of Medicare. You will need to sign an opt out agreement and claims cannot be submitted for services rendered on either behalf. For an explanation please click on Medicare page.

It is the responsibility of the patient to work with their insurance company to determine reimbursement.

Podiatrist in Motion makes no guarantee patient will be reimbursed by their insurance.
